GRANT REQUEST GUIDELINES :

The Citizens Savings Foundation generally gives to tax-exempt organizations whose primary exempt purpose is to provide education or housing opportunities for disadvantaged individuals.

In addition to completing the enclosed Grant Request Application, the following information needs to be submitted:

• Copy of your IRS 501(C)3 exemption letter.
• Copy of most recent Form 990 filed with the Internal Revenue Service.
• Copy of most recent financial statements.
• Budget of program for which funds are being requested.
• Current list of board members or trustees.
• The organization's budget for the current year.
